What to Expect

Caspar Health is entering its next growth phase. Our digital care is becoming increasingly critical to clinic and hospital operations. At the same time, we are progressing towards medical-device obligations and expanding AI-enabled ways of working.

As CTO, you will own Technology and Engineering at C-level. You will report to the CEO, lead Engineering with full functional and disciplinary responsibility and work as a peer to the CPO.

Your job is not to become the bottleneck or Caspar's most important individual technologist. You will build a strong leadership layer and an Engineering system that moves quickly with security, resilience and evidence. You will stay close enough to technology to judge important decisions, risks and AI-enabled development through current practice.


This position is based in Germany and must be performed, living in Germany. Due to our current payroll and social security arrangements, we cannot offer permanent employment from abroad. Applicants must therefore be able to perform their duties from Germany starting on their first day of work and must already have the necessary authorization to work in Germany.

Your Challenges

  • Translate company and product strategy into a technology strategy with clear capabilities, investments, risks and measurable 18-month proof points.

  • Lead all Engineering teams and Engineering leads, including Platform, Infrastructure, DevOps/SRE, developer experience and Engineering-owned quality automation.

  • Build a strong leadership layer, clear domain ownership, distributed knowledge and resilient internal and external capacity without single-person dependencies.

  • Own architecture, technology choices, reliability, security implementation, technical evidence and reporting, and regulated development for critical clinic and hospital workflows. Keep Caspar ready for deep technical scrutiny with current architecture and ownership evidence, demonstrable Caspar IP ownership, auditable security and compliance evidence, a credible AI story and no critical single-person dependencies.

  • Own the incident system and lead major or trust-critical escalations. You are not the standing first-line on-call engineer. Ensure postmortem actions are completed and verified.

  • Plan the Engineering budget and capacity. Decide within the approved envelope on technology, vendors and build, buy or outsource options, including lifecycle cost and continuity.

  • Shape AI-enabled Engineering through current personal practice. Create measurable leverage without weakening security, maintainability, review quality, traceability or regulated assurance.

  • Make credible commitments with Product. Product owns outcomes and scope intent. You own technical feasibility, capacity and release readiness, and stop unsupported or unsafe commitments.

  • Involve Medical, Quality, Information Security, Privacy and Legal early. Engineering turns requirements into reusable controls, tests and evidence. Independent functions retain review, stop and risk-acceptance rights.

  • Establish clear data contracts with Data. Technology owns production and operational data systems. Data owns semantics, certified analytical datasets, metric governance, analytical quality, experimentation and decision science.

  • Represent Caspar in high-stakes technical discussions with clinics, hospital IT, integration partners, auditors, investors and the board.


What success looks like


In your first year, you will:


  • Build a strong Engineering leadership layer and accountable ownership for critical systems

  • Connect technology choices and investment to company strategy and reviewable proof points, then complete a deep external technical review at customer, auditor or investor standard with no unresolved material findings

  • Improve delivery predictability, release quality, resilience and regulated evidence

  • Make cooperation between Engineering, Infrastructure and independent Security durable and effective

  • Create measurable impact through AI-enabled Engineering without weakening security or quality

Your Profile

  • You have managed multiple Engineering leaders and led a multi-team organization through a meaningful scale or rigor transition.

  • You have owned technology strategy, budget, capacity and build, buy or outsource decisions at executive level, with evidence of their impact.

  • You have led Engineering in regulated, security-critical or high-availability systems. Medical-device or digital-health experience is a plus, not a prerequisite.

  • You have built and scaled cloud-SaaS products, ideally in a regulated environment. Health-tech or medical-device experience is a plus, not a prerequisite. You also bring broad systems depth across backend services, web and mobile clients, APIs and integrations, data flows, observability, security and operations. Our exact stack is not a hard filter.

  • You currently work hands-on with agentic development workflows and can show how you used, constrained or rejected AI output and made it scale

  • You have shipped products that achieved meaningful customer and business outcomes and bring product judgement and pace, not only engineering rigor. You can challenge Product constructively, make business and technical trade-offs explicit and protect credible commitments under pressure.

  • You build trust with teams, customers, partners, auditors, investors and boards without acting as a rescuer or creating dependence on yourself.

  • You are business-fluent in German and English.


A university degree or fixed number of years is not required. We care about demonstrated impact, judgement and responsibility you have personally carried.
